Background: The ‘Japa’ syndrome, derived from a Yoruba expression meaning to flee or escape, has evolved from colloquial slang into a recognizable structural pattern of mass emigration among skilled Nigerian healthcare professionals to high income countries.

Purpose: This systematic review investigates the domestic push factors and international pull factors driving this outward migration of health workers, synthesizes evidence on its cumulative impact on Nigeria's healthcare delivery system, and critically appraises intervention strategies aimed at stabilizing the national health workforce.

Methods: A systematic search was conducted in PubMed, Google Scholar, ScienceDirect, and African Journals OnLine (AJOL) for peer reviewed studies published between 2020 and 2026. Data extraction focused on economic, structural, infrastructural, and psychosocial determinants of migration, alongside clinical performance indicators and health policy models.

Key Findings: The domestic context is characterized by intense macroeconomic pressures, including high inflation, persistent currency devaluation, degraded clinical infrastructure, and pervasive insecurity. These factors interact with strong international pull dynamics such as markedly higher remuneration, structured career trajectories, and relatively stable social environments in destination countries such as the United Kingdom, Canada, and the United States. Quantitative estimates indicate that more than 15,000 physicians and several tens of thousands of nurses have emigrated over a five year period, distorting clinician to patient ratios well beyond World Health Organization (WHO) recommended thresholds. The resulting workforce deficit has precipitated severe burnout among remaining clinicians, lengthened patient waiting times, and compromised clinical safety, thereby contributing to increased avoidable hospital mortality. Legislative attempts to retain professionals through coercive or restrictive regulations have encountered strong opposition and have not addressed underlying systemic drivers.

Conclusion: Sustainable workforce retention will require a departure from

restrictive, coercive mandates towards integrated economic and institutional reforms. Priority measures include inflation indexed salary restructuring, substantial hospital modernization, and transparent, merit based career progression pathways. In addition, reframing policy from a narrow ‘brain drain’ narrative to a ‘brain circulation’ paradigm operationalized through formal diaspora engagement frameworks (‘Japada’) offers a scalable mechanism to channel global capital, technical expertise, and knowledge flows back into Nigeria's health system.

Dental anxiety affects approximately 72.6% of the adult population in the United States, with severe fear reported in 26.8% of individuals; this condition contributes directly to treatment avoidance and deteriorating oral health outcomes. Office-based oral conscious sedation (OCS) using triazolam (Halcion) represents a widely adopted pharmacological strategy for managing patients with significant anxiety in high-volume clinical environments, yet systematic documentation audits in private-practice settings remain scarce in the peer-reviewed literature. This retrospective clinical audit examined 39 deidentified operative records of adult patients who underwent OCS procedures with triazolam in a private dental office, assessing guideline concordance against American Dental Association (ADA) 2024 and American Association of Oral and Maxillofacial Surgeons (AAOMS) ParCare 2023 standards. Documentation completeness ranged from 71.8% (adverse event notation) to 100% (drug dose and lot number), with a mean overall concordance of 90.1%. No respiratory or cardiovascular adverse events were recorded. Case acceptance rates increased by 29.4 percentage points following systematic OCS protocol integration. This audit proposes a structured sedation documentation checklist as an original quality-improvement instrument for office-based dental practice. The findings are of direct relevance to general practitioners, dental educators, and policy-makers engaged in sedation safety and clinical governance.

Open defecation remains an important sanitation challenge in Nigeria, and the presence of household latrines does not necessarily indicate consistent latrine use. This paper examined the relationship between household latrine ownership and self-reported open-defecation practice in Okada, a suburban community in Edo State, Nigeria, while also describing perceived conditions associated with continued open defecation. A community-based cross-sectional survey was conducted among adults aged 18 years and above. Of 288 questionnaires distributed, 254 were returned and analysed, giving a response rate of 88.2%. Data were collected using a structured self-administered questionnaire and analysed using descriptive statistics, cross-tabulations and Pearson's chi-square tests. Open defecation was reported by 134 respondents (52.8%), while 214 (84.3%) reported having a latrine at home. Among respondents with household latrines, 121 of 214 (56.5%) reported practising open defecation, compared with 13 of 40 (32.5%) respondents without household latrines. Household latrine ownership was significantly associated with open-defecation practice (χ² = 7.816, p = .005). This association indicates that latrine ownership and consistent avoidance of open defecation did not coincide within the surveyed population and should not be interpreted as evidence that latrine ownership promotes open defecation. Frequently reported perceived explanations for continued open defecation included lack of water or dirty toilet facilities (93.3%), unhygienic toilet conditions (92.9%), lack of toilet facilities (92.1%), poverty (91.3%), long distance from toilet facilities (86.6%) and the perception that open defecation was of limited importance (76.4%). Perceived low importance was also significantly associated with open-defecation practice (χ² = 9.937, p = .002). The findings identify a clear sanitation access-use gap and show that household latrine ownership alone is an incomplete indicator of sanitation behaviour. Sanitation programmes should therefore assess not only facility availability, but also functionality, accessibility, cleanliness, water availability and sustained use.
